Patients
As
a patient, you are a member of the healthcare team. As part of this
team, you play an important role in your safety when receiving
healthcare either in a hospital or clinic. MPSC has worked to
develop the following tools for patients to use to help you be informed
and involved in your care.

Medication brochure and wallet card
Patients are frequently asked by their physicians, “What medications are
you currently taking?” You know that it is extremely important for your
safety to answer this question with the same medications every time, but
it’s easy to forget the exact name or the exact dose for everything you
take. With this in mind, the MPSC developed a brochure called “A Guide
to Prescription Health.” This guide was created for you as consumers to
use safe medication practices in various healthcare locations and in
your home. The brochure was developed with the Pharmacy Society of
Wisconsin. It offers tips on talking with providers and pharmacists,
asking important questions, and how you can maximize your safety when it
comes to medications. The brochure also includes a tear-out wallet card
designed so that you can list and keep track of all of your medications.
Bring the wallet card along on visits to all healthcare facilities,
whether the hospital, clinic or pharmacy.

Patient information and tip sheets
As
part of observing Patient Safety Awareness Week (March 9-15, 2003"Communication
and Partnership: Safety starts with all of us) the
MPSC developed patient tip sheets on how you and your family can partner
with healthcare professionals to make to make your health care
experience safer.
